Lisa Allen, CO Gold, would take over. She is the only executive to survive intact all of David Miscavige's SP declares and offloads. She has a reputation for anti-social conduct, and may be as much a psychopath as Miscavige is. Very little would change. It would literally be "business as usual" in corporate scientology.
True -- she was chosen for the job by winning the game of musical chairs, but from CO Gold to in charge of all Scientology, CST, RTC, CMOI, ASI, Exec Strata included...? It's a bit of a stretch, but not impossible, for she is de facto a direct junior of DM's. (Anyone know if she was ever in RTC?)
My guess for who would take over if DM took off would have been ex-CO CST, Russ Bellin, but that was in 2006. DM has had two years to wipe out any resemblance of authority Russ had, which might make it impossible now.

Probably largely matches DM's list of the most dangerous rivals.
Couple of more prospects -- the only ones DM "trusted" (wrong word for it, "used" is closer) in early 2007:

Legally CST would be the one to take over -- but only if DM doesn't run off with the legal ownership of everything having to do with Scientology, which he is obviously set up to do, with undated signed resignations and a computerized database of blackmail material on all potential rivals included.
Just about the only "salvation" for CoS would be to deliver DM to the authorities themselves and declare him. Problem is, it is set up so that it is impossible to do so from the inside.
(And if you expect the incumbents of the SP Room to ever rise up, you're dreaming. One critical comment about DM from one of them would result in an immediate revocation of any "progress" they have made on their program and any hope for them to ever get out of there.)
All in all, it is still mostly up to DM whether the CoS is going to end with a bang or with a whimper.
